Current medical management of older adults who are living longer with higher cardiovascular risk has caused a reassessment of the character of exercise in congestive heart failure (CHF) Scientific evidence has demonstrated significant improvement in cardiopulmonary function, functional status, and psychosocial status of individuals with heart failure. It also has been documented that patients with CHF have abnormal hemodynamic replications to exercise making it difficult for physical therapists to lay open exercise programs for these individuals. This makes assessing and progressing the exercise prescription difficult at best. This paper provides an overview of the following: underlying disease mechanisms of CHF; the medical management of CHF; the scientific evidence validating the altered hemodynamic replys to exercise seen in patients with CHF and its influence forward exercise prescription, exercise monitoring, and exercise progression. In addition, guidelines are provided for physical therapists to assist when prescribing, monitoring, and progressing exercise programs for patients with acute/chronic heart failure or when CHF is a concomitant disease complication.
