Purpose: The number of geriatric patients undergoing multiple cardiac surgical interventions is steadily increasing each year. These geriatric patients ofttimes have multiple comorbidities, as well as a high risk for post-operative complications. As a accrue these patients require extensive medical and rehabilitative care following acute care hospitalization. This presentation will discuss the physical therapy intervention provided within a station acute inpatient rehabilitation program specializing in the care of geriatric patients' status employment complex cardiac surgical intervention. Description: This presentation will: a) identify the archetypes of cardiac patients benefiting from a seat acute inpatient rehabilitation program; b) describe specific physical therapy intervention incorporating tangled skein medical presentation with functional mobility training and cardiac rehabilitation; c) review program result data for patients classified by dint of the following surgical procedures: coronary artery bypass graft, valve replacement, cardiac arrest/myocardial infarction. Program issue indicators include length of stay, percent of community discharges, and motor FIM gain by stay; d) discuss the prosperous utilization of an inpatient rehabilitation setting in the continuum of care for these images of patients. Summary of Use: Madonna Rehabilitation Hospital created a specialized cardiac program within a long-term acute care hospital (LTACH) unit in july 2002 Since the disclosure of this program, we have been able to promote the needs of geriatric patients who require intensive ongoing medical care in addition to cardiac and physical rehabilitation. Our data supports that this original of program is a critical grade in the continuum of care required to achieve an optimal on a level of function for patients with webwork medical and rehabilitative needs. Importance to Members: The increase in the number of cardiac surgical deeds as well as the ongoing limitations in acute care hospital extents of stay mandate that healthcarc providers address the unique urgencys of geriatric patients requiring composite medical and rehabilitation services. This presentation will discuss the physical therapist's part and interventions within a program designed to confront patient needs following complex cardiac interventions.
