Lower Pulmonary Diffusing Capacity ...Lower Pulmonary Diffusing Capacity in the disposed Vs. Supine Posture. Peces-Barba G Rodriguez-Nieto MJ Verbanck s Paiva M, Gonzalez-Mangado N. Reprinted with permission from J Appl Physiol. 2004;96(5):1937-1942 ?© 2004 American Physiological Society. We evaluated the force of prone positioning on gastransfer characteristics in normal human make submissives Singlebreath (SB) and rebreathing (RB) maneuvers were give employment toed to assess carbon monoxide diffusing capacity (DL^sub CO^) its composings related to capillary blood contortion (Vc) and membrane diffusing capacity (Dm) pulmonary tissue whirl (Vti), and cardiac output (Qc) Alveolar book (VA) was significantly greater recumbent than supine, irrespective of the criterion maneuver used. Nevertheless, DL^sub CO^ was consistently lower inclining than supine, a difference that was enhanced when appropriately corrected for the higher V^sub A^ disposed When adequately corrected for V^sub A^, diffusing capacity significantly decreased according to 8% from supine to inclined [SB: DL^sub CO,corr^) supine v prone: 326 ?± 23 (SE) v 300 ?± 2 ml min^sup -1^ . mmHg^sup -1^ STPD; RB: DL^sub CO,corr^ supine v prone: 302 ?± 22 (SE) v 278 + 20 ml ?· min^sup -1^ mmHg^sup -1^^sub stpd^]. Both VC and Dm showed a drift to decrease from supine to inclined but neither reached significance. Finally, there were no significant differences in Vti or Qc between supine and propense We interpret the lower diffusing capacity of the healthy lung in the prostrate posture based on the relatively larger space occupied at the heart in the pendent lung zones, leaving less space for girdle 3 capillaries, and on the relatively lower position of the heart, leaving the girth 3 capillaries less engorged. Gas Exchange During Separate Diaphragm and Intercostal Muscle Breathing. DiMarco AF, Connors AF Jr Kowalski KE Reprinted with permission from J Appl Physiol. 2004; 96(6):2120-2124 ?© 2004 American Physiological Society. In patients with diaphragm paralysis, ventilation to the basal lung girdles is reduced, whereas in patients with paralysis of the rib cage muscles, ventilation to the upper lung surface bounded by parallel circles s is reduced. Inspiration produced according to either rib cage muscle or diaphragm contraction alone, therefore, may outcome in mismatching of ventilation and perfusion and in gas-exchange impairment. To standard this hypothesis, we assessed gas exchange in 11 anesthetized dogs during ventilation produc at either diaphragm or intercostal muscle contraction alone. Diaphragm activation was achieved at phrenic nerve stimulation. Intercostal muscle activation was accomplished by way of electrical stimulation by using electrode positioned epidurally at the T^sub 2^ spinal cord on a level Stimulation parameters were adjusted to provide a constant tidal body and inspiratory flow rate. During diaphragm (D) and intercostal muscle breathing (IC), mean arterial PO^sub 2^ was 971 ?±21 and 881 ?± 27 Torr, respectively (P Prognostic Value of Stres Testing In Patients throughout 75 Years of Age With Chronic Angina. Jcger RV Zellweger MJ Kaiser C Grize L Osswald s Buser PT, Pfisterer ME; TIME Investigators. Chest. 2004;125(3):1124-1131 ?© 2004 American corporation of Chest Physicians. Study Objectives: To define the prognostic value of stres testing (STRT) in patients ?‰? 75 years of age. Design: Multicenter prospective randomized trial. Setting: Tertiary care center Patients: sum of two units hundred ninety-two patients of the Trial of Invasive v Medical Treatment of somewhat old Patients aged ?‰? 75 years with chronic angina despite receiving pair or more antianginal drugs were prospectively observ for 1 year. Intervention: STRT (88% exercise ECG; 12% pharmacologic stres imaging) was performed if possible, and ischemia was diagnosed using in every one's mouth guidelines. Death for any reason and nonfatal myocardial infarction were consequence events. Results: Patients who could perform STRT (148 patients) were younger, had a lower risk profile, received les medication, and had les simple angina than patients who could not perform STRT (144 patients). The 1-year mortality rate was merely 1.4% in patients with negative STRT deductions (72 patients) compared to 53% in patients with positive STRT follows (76 patients) and 13.7% in patients who had not undergone STRT owed to unstable symptoms (95 patients). The corresponding 1-year rates of death/infarction were 28% 158% and 263% respectively. After adjustment for baseline differences, mortality rates were no longer significantly different. However, compared to patients with negative STRT springs infarction and death/infarction rates remained higher in patients with provocable ischemia (hazard ratio [HR] 89 [p = 004]; HR 61 [p = 002] respectively) and in patients without STRT befitting to unstable angina (HR, 118 [p = 002]; HR 86 [p =004] respectively). Conclusions: STRT in somewhat old patients is feasible and provides important prognostic information for their coming events management. Patients with negative STRT arises after receiving therapy have a serviceable prognosis, and their conditions may be managed conservatively. |
