ABSTRACT

Purpose: This consideration was designed to compare balance performance, perceived benefits, and compliance rates of community-dwelling older adults who participated in a short-term computerized balance training program (CBT) a abiding-place program of balance exercises (HEP), or no program. Methods: Eighty-eight controls were randomly assigned to train interactively forward the Biodex Balance System, train at place of abode using an illustrated exercise program, or perform no training for a period of 4 weeks. Pre- and post-test measures included simple reaction time (SRT) timed 50-foot walk touchstone (TWT), the Berg Balance Scale (BBS) and the Modified Falls Efficacy Scale (MFES) Results: A 3 ?— 2 analysis of variance (ANOVA) revealed no significant differences in the performance improvements observ among the 3 disposes (over time) in any of the measures. Likewise, an independent t-test showed no difference in compliance rates between the pair training groups. Chi-square analyses of subjective annotations indicated that subjects in the CBT arrange found their training to be more interesting and challenging than those who trained with the HEP. Approximately 20% of bring under rules in each group reported that their balance had improved. Conclusion: Although compliance was profitable in both training groups and many exposes perceived an improvement in their balance and/or functional abilities, neither pattern of short-term balance training improved performance measures to a significant compass when compared to control subjects

INTRODUCTION

Various images of exercise interventions have been introduced throughout the past decade as an approach to fall prevention/reduction in older adults. The efficacy of many of these interventions was studied in a settle of clinical trials known as the FICSIT subject of attention (Frailty and Injuries: Cooperative Studies of Intervention Techniques). The duration of these interventions ranged from 10 to 36 weeks. A meta-analysis of the data accumulateed from 7 study sites indicated that exercise interventions generally reduc falls at 10% to 20% (compared to nonexercising manage groups) with higher reductions coming from programs that included balance training.1

Many balance training studies including 2 of the FICSIT trials have focused upon the use of computerized devices, like as the Balance Master' (NeuroCom International, Inc.,Clackamas, OR), that provide visual feedback to make liables regarding the position of their center of gravity relative to the support surface (ie, force platform). This image of biofeedback has been advocated for many years as an adjunct treatment for patients with impaired neuromuscular function proper to cerebral palsy, stroke, or vestibular dysfunction.2 Numerous studies upon patients with hemiplegia have documented the consequence efforce platform biofeedback on various component parts of balance including steadiness (minimal postural sway), weight-bearing proportion and dynamic stability.3 For example, Shumway-Cook et al reported greater reductions in postural sway among patients whose physical therapy was augmented according to balance training when compared to patients who received traditional physical therapy alone.4 Likewise, Winstein et al fix greater improvements in static weight-bearing proportion among patients whose physical therapy included visual feedback plane though these improvements did not bring to asymmetrical limb movements during gait.5

Nichols reviewed balance retraining studies that involved hemiplegic patients and cited inconsistent be the effects regarding the functional benefits of computerized balance training.3 However, she conclud that these biofeedback protocols add variability to the treatment session and, when combined with other activities, frequently improve the patients' functional abilities. Several studies that have focused in succession the benefits of balance training in combination with other interventions, as it was as strength training, seem to support her conclusion. In the same of the FICSIT trials, investigators compared changes in might balance, and walking speed among older adults who received power training, balance training, combined force and balance training, or no training.6,7 Balance training was performed in succession a computerized balance platform first with visual feedback and later with no feedback. power training was performed with liberated weights and variable resistance machines. Balance training and combined training significantly improved all balance measures, while vigor training only improved measures of muscle strength

Verfaule compared the purports of a strength training program to a combined solidity and balance training program and originate that the addition of balance training improved balance and gait measures beyond the improvements achieved from might training alone.8 Although this research also used resistive equipment for impregnability training, a low-tech approach was used for balance training. This approach included a combination of Tai Chi, low-impact aerobic dance manner of movings gait training on an obstacle course, and traditional balance exercises. Examples of balance exercises include weight-shifting, standing upon one leg, tandem walking, and crossover walking (braiding). Lichtenstein et al also reported onward the effects of a simple exercise program designed to improve flexibility, balance, and reaction time among a collection of older women living in an apartment complex9 Inconsistent differences were place between exercise and control subjects; however, those who had high compliance rates (>85%) demonstrated greater improvements than those who had subdued compliance.
